Work Schedule Template for Multiple Employees
This form is to be used by supervisors to approve a flexible work schedule which permits exceptions to the normal hours of operation. Flexible work schedule are required to be completed in advance of the change under most circumstances.
Work Schedule: is the planned hours of work over a period of time such as day(s), week(s), month(s). Work schedules shall be repeated on a continuous basis, subject to change in accordance with the BPA/CPTC Collective Agreement. Included within a work schedule is the shift(s)/tour(s) of duty the employee is expected to work.
